  We at The Pain Institute in Chicago have been developing a treatment
protocol for chronic pain conditions such as Reflex Sympathetic Dystrophy
and Fibromyalgia that combines hypnosis, massage therapy, and conventional
medical treatments.  We are interested in being listed on a referral service
