This is the Food and Drug Administration MedWatch service.
 
Let the voice mail default to a human being, and ask to be mailed
the paper work to report.
 
They will ask for a snail name and address.  They will ask if you
are a consumer; yes, unless you are writing prescriptions.
 
I would think one could also report adverse reactions to shock,
which FDA classifies as medical devices.
 
I would expect they only want to know about recent reactions.
